What do I need to know before scheduling a physical, pap exam or well child check?

All physicals, pap exams, well child checks and sports physicals will be billed as preventative care. It is the patient's responsibility to check with their insurance company for covered benefits. We request that you sign a waiver if you are scheduling any of the above listed exams.

How do I arrange a medical referral?

REFERRAL POLICY

It is our policy to treat all illnesses and injuries as appropriate. Referrals are given at the discretion of your primary care physician. If you are referred to a specialist, be sure and verify with your insurance company that the specialist is covered under your plan. It usually takes 7 to 10 working days for your insurance company to mail confirmation of the referral to you.

How much do office visits cost?

Cost varies by depending on the reason for the visit.
The average cost is approximately $110.00 for the office visit.
There may be additional costs for labs or medical supplies.

How much is my co-pay?

Your co-pay varies depending on the type of insurance you have.
To find out what it is look on your insurance card.
If it does not specify on your card, there is no co-pay.

If there is a cost, how do I make payment?

We accept credit cards, checks, and cash.
 
Payment must be received upon conclusion of visit.

Do I have to pay for my visit at the time of my appointment?

We require that you pay your co-pay at your appointment.
Only on specific occasions do we require you to pay for the visit at the time of your appointment. Such occasions would be if you are visiting the area and you have no insurance.
If you do choose to pay the full cost of your appointment on the day of your visit, there will be a discount applied.

Will insurance pay for everything?

In some instances yes, your insurance company may cover the all cost.
Please contact your insurance company to see if they will cover the costs and the appropriate steps you will need to take if they don't.
