Reason for appointment
Patient's day time and evening phone number.
If you need to cancel an appointment, please call as
soon as possible.
This way, we can meet the needs of other patients during your cancelled time.
After hours, Dr. Karen and Kelly Locke will be available by
phone to handle urgent medical needs.

Motor vehicle accidents are billed directly to the patient, and
the patient then needs to file the expense with their auto insurance carrier.
Please make sure you are specific when scheduling appointments that you state it
is an MVA related appointment.
Dr.'s Locke do not take Workmen's Compensation
insurance (i.e. Pinnacol).
If you are a patient of Dr. Locke, we can suggest options for physicians who
accept Workmen's Compensation insurance.
If you feel that your condition requires an antibiotic then you must schedule
an appointment to be seen. Antibiotics have become resistant to some bacteria
and we feel it is important that you be evaluated before we will prescribe an
antibiotic.
LFM REFILL POLICY
If you need a refill on a medication, please call your
pharmacy where you have the medication filled, and they will fax us the request.
It is important to give us at least 24-48 hours to process the request, so do
not wait until you are completely out of the medication before requesting a
refill.
PAIN MEDICATION
REFILLS
Pain Medicines will only be refilled Monday - Friday
from 8:00am to 5:00pm with the same guidelines as stated above. The pharmacy
will fax us the request, allow at least 24-48 hours to process the request.
It will also only be refilled if your Primary Care
Physician is in the office (excluding days off for vacations).
PROCESS TO REQUEST CHART
COPIES/MEDICAL RECORDS
- Signed authorization to transfer medical records is required for all
requests.
This can be accomplished :
- In person
- By mail (our address is 1450 East Valley Road, Ste 101, Basalt, CO
81621)
- Or by fax. Our fax number is 970-927-3915.
- Charts are copied as quickly as possible, but may not be ready for 1-3
days.
- If only a page or two are required from the chart we will copy it here,
with a signed medical information release. Extensive copy requests may have an
associated fee to complete.
- Storage - some charts may need to be requested from storage, if non-urgent
we will request these charts once per month, if urgent we will send special
request (fee associated)
- Chart copies are provided within 30 days after receiving a completed
medical release
Locke Family Medicine (LFM)
is
a private Limited Liability Corporation owned by the physicians within the group.
LFM is not directly affiliated with Aspen Valley Hospital (AVH) in Aspen, Colorado.
LFM physicians
are on staff at AVH and admit patients to this hospital for care. We utilize AVH resources such as
the hospitalist, specialists, special testing, ER care, etc,
but we also
utilize medical resources at Valley View Hospital (VVH) in Glenwood Springs, Colorado,
when necessary for the care or convenience of our patients.
However, we do not currently have admitting privileges at VVH.
