  Insurance Plan Participation and Billing

The following list includes the major Insurance Plans in which Locke Family Medicine Physicians participate.

This list may change from time to time, so please check with the Practice when you make your first appointment.

Aetna/Sloans Lake -- If your insurance card says Cofinity or Sloans Lake, we accept the insurance.
Common insurances that might include this logo are: Aetna, American Medical Security, Assurant Health, CNIC, Doc First, Golden Rule, John Alden, MegaLife, Pacificare, Principal, etc.
     Note: If your insurance card does not have the Cofinity or Sloans Lake logo on the card, then we are not part of that insurance’s network of providers.

As a courtesy, we also bill insurances with whom we do not participate.
However, payment in full is expected at time of visit.

Note: Discounts for Payment at Time of Service only applies if the visit is paid in full and Locke Family Medicine doesn't bill the insurance.

We encourage you to become familiar with the policies and guidelines of your health insurance provider. Our staff will prepare and submit the proper forms on your behalf and will bill you for any amount not covered by your insurance carrier if we are participating in the plan.

If you experience a financial hardship, please contact our Billing Office at 970-279-4111 to arrange an individual payment plan.
